Mission: The mission of sears is to provide leadership in response to the needs of our aging population. We believe we have a responsibility for studying, analyzing, planning, developing, and implementing quality service to the aging where appropriate needs are found for health care and residential living services and can be provided in a christian environment. Programs: Sears brazos retirement corporation, a non-profit 501(c)(3) corporation, owned and operated wesley woods alzheimer's care center in waco, texas. Wesley woods was a 120 bed nursing facility. The facility provided nursing home and dementia care operations. The facility also provided long-term nursing care to the elderly as well as a medicare part b care for rehabilitation. In 2011, the system sold the assets and ceased operating wesley woods. The proceeds of the sale of the property were used to pay down debt with the remaining proceeds paid to the system to pay transaction costs and provide additional cash to the system.

Programs: The organization provides long-term nursing care to elderly who are no longer able to care for themselves or elderly who may have different stages of dementia. Assistance is also provided to senior citizens who are still somewhat independent with their daily lives - offering services such as meals, housekeeping, and the availability of nursing staff. Other services provided include home health & hospice services to residents in our long term care communities as well as the outside community. Also provide specialty assistance such as 24 hour care, assistance for personal needs, & home health maintenance. Therapy services can also be provided to residents of our communities.

Programs: Sears panhandle retirement corporation is a non-profit 501(c)(3) organization, which consists of 173 independent living and cottages and 40 assisted living units as well as a 120 bed long term care skilled nursing and dementia care community. The facility provides nursing home and dementia care operations. It also provides long-term nursing care to elderly as well as a medicare skilled unit for rehabilitation.

Programs: Sears plains retirement corporation, dba: garrison geriatric education and care center owns and operates a senior living facility. The facility provides nursing home and dementia care. It also provides long-term nursing care to elderly as well as a medicare skilled care unit for rehabilitation. The facility consists of a 120 bed skilled nursing facility and also serves as a place of instruction for health students from the texas tech health sciences center.

Mission: Assisted living services for disadvantaged members of senior population. Programs: Assisted living services for disadvantaged members of senior population. Services include community development, assisted living, adult daycare, health counseling, transporation services, recreation, and life skills training.

Mission: Bridges care community is a catholic organization entrusted with advancing the health care ministry of the benedictine sisters of duluth, minnesota. Our mission is to witness to god's love by providing compassionate, quality care with special concern for the underserved and those in need. Programs: Operation of bridges care community, is a 49 bed long-term care facility in ada, minnesota which provides skilled nursing care to the elderly. The facility had 92% occupancy and served 92 individuals during the fiscal yearending 6/30/17. operation of benedictine living community of ada, a 18 unit assisted living facility in ada, minnesota. The facility had an average occupancy of 96% and served 29 individuals during the fiscal year ended 6/30/2017.

Mission: To provide quality residential housing for senior citizens along with personal services, assisted living and nursing services. Programs: Nursing and retirement facility with services ranging from individual retirement living to 24-hour skilled nursing care.

Mission: Provide a therapeutic living situation for persons over the age of 18 years that require care at a residential care facility level due to physical or mental disabilities which respond best to structure and supervision in their daily lives. Programs: Provided medical attention, vocational and environmental support to improve the quality of life and self-esteem of each individual serviced and return the individual to their highest possible level of functional independence to 80 residents. day habilitation services are provided on a daily basis. This program focuses on social skills, communication skills and behavior management. This service is provided at our facility. home based habilitation - we are providing staffing and services for group homes for individuals with functional impairments. home-based habilitation - we provided a limited amount of services for clients with functional impairments that live in the community but are not in one of our group homes. This may include spending time with a client working on medication management, budgeting, grocery shopping and other life skills.
