Mission: Able inc is dedicated to enhancing relationships,providing opportunities for growth and encouraging people to reach their personal dreams. Programs: Provides day supports/employment opportunities for 75 individuals with intellectual disabilities, provide residential support in msla/icf for 57 individuals with intellectual disabilities and isla/sla/family support - supported 15 individuals for independent living.

Mission: NEBA provides excellent venues both locally and internationally for the exhibition of the accomplishments of legally blind artists from across the local area and recently, the world.

Mission: The arc of rensselaer county is committed to advocating for the rights and safety of individuals of all ages who primarily have intellectual disabilities and, in some instances, have other developmental disabilities or delays. The arc is a source of support, specialized knowledge, and resources in assisting individuals and their families to determine their needs and goals. The arc also works to influence societal attitudes and resources so that people with intellectual and developmental disabilities will have access to the services, assistance, and opportunities they need for personal growth and development and participation in the community. Programs: Community residence - provides support to adults in housing and home life in a variety of settings, ranging from 24-hour supervised settings to supportive apartment living, to a small shared living experience with people with or without disabilities. Number of individuals served - 203. 68,961 units of service. riverside enterprises - this is the vocational training component which includes the following: supported employment for adults in various job settings throughout the capital district. Vocational services for adults who perform work in packaging products and wire forming. A school-to-employment program assists students from three school districts as they transition from the public school system into employment. Number of individuals served - 381. 18,127 units of service. brunswick center services - provides day programming for adults involving a wide variety of opportunities within the community as well as physical, speech and occupational therapy, prevocational services, geriatric services, and independent living. Number of individuals served - 141. 21,834 units of service. community inclusion project - provides individually designed opportunities for adults in various sites in the community. Number of individuals served-56. 17,436 units of servicefamily support services - provides support to families with a child, adolescent, or adult who has a developmental disability. Among the services provided under family support are: services coordination, respite and habilitation services, recreation and summer camp activities, assistance with guardianship, and family reimbursement fund to meet needs that are not supported elsewhere. Number of individuals served - 142 . 101,314 units of serviceself determination/agency sponsored community habilitation - supporting individuals with self-directed services in a variety of roles; ranging from planning, program management, fiscal management, and evaluation. Number of individuals served - 25. 47,420 units of servicemedicaid service coordination - service which assists individuals and families in gaining access to necessary services and supports appropriate for the person's needs. The program uses a person centered planning process in developing, implementing, and maintaining an individualized service plan (isp) while promoting the concepts of choice, individualized services and supports, and consumer satisfcation. Number of individuals served - 289. 2,725 units of serviceboard designation - activities that are components of the agency mission statement but are not part of government funded programs, for instance grants where the agency is the fiscal agent, cobra management for former employees and other board directed activities.

Mission: The kelberman center is a regional center for excellence for individuals with autism spectrum disorders and related learning challenges. We are dedicated to excellence in service through: prompt evaluation and diagnosis; individualized education and services; social and life skills enhancement; innovative practices, training and research. We believe in the power of individuals, family, and community working together to maximize our collective potential. Programs: Individualized residential alternative (ira): during 2013, the kelberman center, inc. Opened two iras in chittenango, new york. This residential program focuses on individualized services that promote independence. Adults living in the ira focus on building and enhancing skills for everyday living such as cooking, cleaning, laundry, hygiene, etc. Individuals also have the opportunity to be involved in community volunteer work and/or supportive employment opportunities, as well as increased recreation opportunities in the community. 8 consumers were served during 2014. the promise program: a full-day educational program for preschool children who are typically between 3 and 5 years old. This program incorporates educational services, intensive therapy, and family support for children who demonstrate characteristics related to autism spectrum disorders. The program goal is to promote language development, social skills and academic knowledge in preparation for their transition to elementary school. During 2014; kelberman served 56 participants throughout 24 school districts. program consultation services for school age students with asd: services to support the work of the classroom team in planning successfully for students with autism spectrum disorders. Program consultations may be provided by an individual specialist or by a team of specialists based upon the needs identified in the referral process. A thorough review of the students developmental, academic and social history, classroom observation, functional behavioral assessment and comprehensive assessment may be included. A total of 35 districts served during 2014. other program services include family support services, consolidated supports & services, community social and recreational programs, and professional training and community education.

Deaf & Hearing Impaired Centers, Disabilities, Human Services

Mission: Cleveland hearing and speech center is the premier provider of hearing, speech-language and deaf services, education, and advocacy helping people connect through communication. Programs: The speech-language pathology program provides the following array of services: speech and language evaluation, speech and language therapy to children and adults, speech and language screenings at head starts and other community venues, and other services related to speech and language disorders. Cleveland hearing and speech center (chsc) has an affiliation with case western reserve university (cwru) in which chsc assists in training masters' level students in the field of speech-language pathology. Cwru rents a significant portion of space from chsc in furtherance of this affiliation. the audiology program provides the following array of services: hearing loss prevention, hearing screenings, hearing evaluation, dispensing hearing aids, reduced cost hearing aids to needy individuals, assistive living technology consultation and sales, and corporate wellness consultation. Chsc provides consultation and educational services to families who have infants that have been diagnosed with severe hearing loss through the ohio department of health regional infant hearing program. the community center for the deaf and hard of hearing provides the following array of services: peer support/case management, consumer education, american sign language classes, community and educational sign language interpretating, c-print services, assistive device consultation, video relay services (vrs), neuropsychology evaluations, and signstage educational programming.
